ˇ·Application Rules and Forms
1. The follow options are available for entering the ICLA:
1-1. Self-nomination
1-2. Nomination: Be submitted by the members of the public/groups.
1-3. By Invitation:
Individuals and groups with outstanding accomplishments and made important contributions to the nation and society may be invited by the organizers.
2. Please attach two copies of the following information with the application. The organizers may disqualify a candidate if they are found to be ineligible.

2-2. Documentary Proof
2-2-1. Individual award candidates should include a photocopy of their citizen ID's front and reserve sides. (Foreigners should provide a photocopy of their passport).
2-2-2. If the individual award candidate is mentally or physically handicapped, please include a photocopy of their handicapped handbook.
2-2-3. Group award candidates must provide documentation such as proof of registration.
2-3. Recommendation Letter (only required for nominated candidates) (Please download)

2-5. A copy of their Autobiography or Group background: (Please download)
2-5.1. The autobiography should include their academic background, outstanding achievements, as well as their important role/contribution to society in the field of caring for life. The relevant certificates, documents and supporting information should be included as well. (e.g. publications, audio-video CDs, certificates, newspaper cippings, video tapes etc.)
2-5.2. The Group background introduction should include the group history, services, outstanding achievements and their contribution to the nation or society. Please include the relevant supporting documentation as well (e.g. publications, audio-video CDs, certificates, newspaper clippings, video tapes etc.)
3. All entries are to be submitted by mail. Please send the application details (in 2 copies) by registered post to the organizers before July 31 2008 (based on postmark. Entries after the deadline will not be accepted).
ˇ·Eligibility
1. Individual Awards: Those with 3 years of experience. Open to all genders and ages. The deceased (including their posthumous works) are not eligible for nomination.
2. Group Awards: Candidates must be officially registered groups.
3. Special Contribution Award: Nominations for this award will be determined by the organizers at its discretion. (One outstanding individual will be selected by the organizers from global suicide prevention and caring for life related fields)
ˇ·Selection and Nomination
Nominations will be accepted before the deadline from public individuals and/orgroups. These should include a description of their accomplishments and supporting documentary evidence. The nomination method is outlined below:
(1) Applications are open to the public. These may be in the form of nomination, self-nomination and organizer's invitation. Entries will be sought through the public broadcast media and the general public encouraged to nominate individuals or groups eligible for the awards by submitting their accomplishments and relevant details to the Foundation using the application forms.
(2) Please use the specified application forms (please download)
Two (2) copies should be provided .Information should include not only their accomplishments , the nominating unit and general descriptionss but also the relevant certificates, newspaper clippings and copy of their citizen ID or business registration.
(3) All entries are to be submitted by mail. Please send the application details (in 10 copies) by registered post to the organizers before July 31 2007. (Based on postmark. Entries after the deadline will not be accepted.)
